Connectedness to Nature Survey
Please fill this questionnaire out before you begin the program and then after every four
activities or trails. Please ask each participant to complete a separate questionnaire by
indicating a response from 1 to 5 beneath each question. This will help us evaluate the
primary program aim. Thank you.
